Brevard Health Alliance
Brevard Health Alliance works with a network of community partners to enable Brevard county citizens to
access expanded primary health care, dental health, mental health, health care education and
preventive health services.
As Brevard County's only federally qualified health center, BHA offers primary health care serices to
citizens regardless of their ability to pay for those services. BHA enables access to reduced priced
medications for its patients, as well.
Brevard Health Alliance operates from fixed-site clinics in the cities of Cocoa, Palm Bay and Melbourne.
BHA also operates a mobile health unit that provides on-site primary care to transitional housing
facilities, homeless centers and in-patient treatment facilities throughout central and southern Brevard
County.
Patient Registration- Monday through Friday (8:30am-12noon & 1pm-5pm).
Walk-in service, first come-first served. Important: Registration process required before medical
appointment is scheduled.

Brevard Health Alliance Registration Process
- The patient should have some form of picture ID or other proof of Brevard County residency.
- The patient should bring their Social Security card, if possible.
- If applying for BHA's sliding fee discount scale, proof of gross monthly income for all related
household members should be provided such as, paycheck stubs, Social Security income, bank
statements, and W-2 statements.
- The patient's most recent Federal Income Tax Return is required to apply for assistance with
medications.
